Ariel Rosenbaum, LCSW-S, C-ASWCM, TxCG

Ariel knew early on that she enjoyed working with seniors and helping others. She earned her Bachelor’s degree in Social Work from Georgia State University, along with a Certificate in Gerontology. She went on to complete her Master’s in Social Work at The University of Georgia.

Ariel began her career in Atlanta before relocating to Austin, where she worked as a hospice and home health social worker. In 2010, while continuing her hospice work, she founded Texas Family Services (TFS) to help families navigate senior housing options.

In 2013, Ariel expanded her mission by launching Senior Services of Austin to offer comprehensive geriatric care management. That same year, her grandfather’s illness brought new perspective. Managing his care—coordinating long-term care insurance, private caregivers, and medical needs—highlighted just how vital expert guidance can be for families. This personal experience continues to shape her commitment to compassionate, client-centered support for older adults.

Ariel co-founded Practical Care Continuum in 2020. This home care company complements her care management work by providing dependable, hands-on assistance to help seniors remain safe and supported wherever they consider home. Together, her organizations offer a full spectrum of services designed to meet families where they are—with practical solutions and compassionate guidance.

Erica Schmidt, LCSW-S

Erica is a Licensed Clinical Social Worker and Supervisor who holds a bachelor’s degree in Plan I Honors from The University of Texas at Austin. She also earned a Master of Science in Social Work from the University of Texas at Austin, where she received specialized training in gerontology through the Gerontology Resources and the Aging Community in Education (GRACE) program. With experience in both outpatient and inpatient hospice care, as well as social services within a continuum of care senior living community, she is well-educated and well-resourced to support older adults in the Greater Austin area.

Sara Tucker, LCSW-S, TxCG

Sara is a Licensed Clinical Social Worker and Supervisor who holds a bachelor’s degree in Latin American Studies from Tulane University. She also earned a Master of Science in Social Work from the University of Texas at Austin, where she received specialized training in gerontology through the Gerontology Resources and the Aging Community in Education (GRACE) program. Sara’s experience includes providing care management for older adults and working with people and families impacted by a dementia diagnosis.

Dee Dee Diaz, LCSW

Dee Dee received her bachelor’s degree in Human Development and Family Studies and a minor’s degree in gerontology from Colorado State University. She earned a Master’s of Science in Social Work from The University of Texas at Austin with specialized training in working with older adults through the Gerontology Resources and the Aging Community in Education (GRACE) program. Her experience includes providing admissions and social services within long-term care facilities, assisting with discharge planning in hospital settings, and providing services for individuals and families who are impacted by a cancer diagnosis.

Alyssa received her bachelor’s degree in social work from the University of Texas at Austin. She earned a Master of Social Work degree from the University of Texas at San Antonio. Alyssa’s experience includes providing case management to families and individuals in long-term facilities, and providing individual and group support to those in long-term care

Sydney is a Licensed Master Social Worker who received her bachelors and masters degree in Social Work at Texas State University. She has experience in care management for individuals who have complex needs, with a passion for building strong relationships with clients and families.

Savannah Brinkley, LMSW

Savannah is a Master of Science in Social Work graduate from The University of Texas at Austin, where she received comprehensive clinical training with a focus on aging populations. She also holds a bachelor’s degree in psychology and a minor in sociology from Texas State University, where her academic foundation in human behavior and social systems shaped her commitment to holistic care.

Savannah is passionate about supporting older adults and their families, both personally and professionally. Her experience includes interning in a dementia-specialized clinic, delivering therapy services, and providing care coordination across interdisciplinary teams. She emphasizes person-centered care in all aspects of her work, ensuring that services are tailored to the unique needs, values, and goals of each individual.

In addition to her clinical background, Savannah has demonstrated leadership in care settings, contributing to team-based approaches and supporting efforts to improve service delivery for individuals and families. She is dedicated to promoting dignity, autonomy, and quality of life through compassionate and evidence-based practice.

Katy Wilkerson, LCSW

Katy is a Licensed Clinical Social Worker and a native of Winters, Texas. She earned her Master of Social Work from Texas State University, where she participated in the Behavioral Health Workforce Education and Training program and completed a study abroad experience in Accra, Ghana.

Katy’s professional experience spans hospital, long-term care, and hospice settings. She began her career at Hendrick Medical Center in Abilene, Texas, supporting patients and families through discharge planning and care transitions. She later served as Director of Social Services at Cedar Pointe Health and Wellness in Cedar Park, supporting residents and families within a skilled nursing and rehabilitation setting. Most recently, she has provided hospice support to patients and families throughout the Austin area.

Outside of work, she enjoys traveling and spending time with her kitties.

Nanci W Kahn, RN

Nanci received her RN degree from Harper College in Palatine, Illinois. She also earned a Bachelor’s degree in Psychology from Indiana University. Her entire nursing career has focused on providing quality care to seniors and support to their family members. Her experience extends from hospitals, to skilled nursing facilities, to in home care. Her passion lies with providing comprehensive in-home assessment and care plan to identify the client’s needs working with the client, family members, care givers, and doctors.
