Ariel Rosenbaum, LCSW-S, C-ASWCM, TxCG

Ariel knew early on that she enjoyed working with seniors and helping others. She earned her Bachelor’s degree in Social Work from Georgia State University, along with a Certificate in Gerontology. She went on to complete her Master’s in Social Work at The University of Georgia.
Ariel began her career in Atlanta before relocating to Austin, where she worked as a hospice and home health social worker. In 2010, while continuing her hospice work, she founded Texas Family Services (TFS) to help families navigate senior housing options.
In 2013, Ariel expanded her mission by launching Senior Services of Austin to offer comprehensive geriatric care management. That same year, her grandfather’s illness brought new perspective. Managing his care—coordinating long-term care insurance, private caregivers, and medical needs—highlighted just how vital expert guidance can be for families. This personal experience continues to shape her commitment to compassionate, client-centered support for older adults.
Ariel co-founded Practical Care Continuum in 2020. This home care company complements her care management work by providing dependable, hands-on assistance to help seniors remain safe and supported wherever they consider home. Together, her organizations offer a full spectrum of services designed to meet families where they are—with practical solutions and compassionate guidance.

Sara Tucker, LCSW-S, TxCG

Sara is a Licensed Clinical Social Worker and Supervisor who holds a bachelor’s degree in Latin American Studies from Tulane University. She also earned a Master of Science in Social Work from the University of Texas at Austin, where she received specialized training in gerontology through the Gerontology Resources and the Aging Community in Education (GRACE) program. Sara’s experience includes providing care management for older adults and working with people and families impacted by a dementia diagnosis.

Dee Dee Diaz, LCSW

Dee Dee received her bachelor’s degree in Human Development and Family Studies and a minor’s degree in gerontology from Colorado State University. She earned a Master’s of Science in Social Work from The University of Texas at Austin with specialized training in working with older adults through the Gerontology Resources and the Aging Community in Education (GRACE) program. Her experience includes providing admissions and social services within long-term care facilities, assisting with discharge planning in hospital settings, and providing services for individuals and families who are impacted by a cancer diagnosis.

Alyssa Ybarra, LMSW, TxCG

Alyssa received her bachelor’s degree in social work from the University of Texas at Austin. She earned a Master of Social Work degree from the University of Texas at San Antonio. Alyssa’s experience includes providing case management to families and individuals in long-term facilities, and providing individual and group support to those in long-term care

Amelia Card, LMSW

Amelia Card is a dedicated geriatric care manager and Licensed Masters Social Worker committed to empowering older adults and their care partners through compassionate, evidence-based care. As a Geriatric Care Manager at Senior Services of Austin, she manages complex caseloads and collaborates with multidisciplinary teams to ensure high standards of care, with a special focus on neurocognitive disorders.
Amelia’s career is shaped by a blend of clinical, research, and leadership experience. As a clinical social work intern at the University of Texas Health Austin Comprehensive Memory Center, she provided client-center care and utilized therapeutic modalities such as solution-focused therapy, motivational interviewing, and narrative therapy. She also facilitated therapeutic songwriting sessions, using music as a creative outlet to support emotional expression and connection among older adults. Her research background includes supporting caregiver-engaged fall prevention interventions for older adults with dementia at the Steve Hicks School of Social Work.
Before becoming a social worker, Amelia co-founded Camp Spiritfire, a nonprofit supporting individuals in recovery. She has facilitated grief and bereavement support groups and led creative activities for older adults with cognitive decline. She is passionate about holistic, person-centered care and fostering resilient communities.

Erica Schmidt, LCSW-S

Erica is a Licensed Clinical Social Worker and Supervisor who holds a bachelor’s degree in Plan I Honors from The University of Texas at Austin. She also earned a Master of Science in Social Work from the University of Texas at Austin, where she received specialized training in gerontology through the Gerontology Resources and the Aging Community in Education (GRACE) program. With experience in both outpatient and inpatient hospice care, as well as social services within a continuum of care senior living community, she is well-educated and well-resourced to support older adults in the Greater Austin area.

Nanci W Kahn, RN

Nanci received her RN degree from Harper College in Palatine, Illinois. She also earned a Bachelor’s degree in Psychology from Indiana University. Her entire nursing career has focused on providing quality care to seniors and support to their family members. Her experience extends from hospitals, to skilled nursing facilities, to in home care. Her passion lies with providing comprehensive in-home assessment and care plan to identify the client’s needs working with the client, family members, care givers, and doctors.
